We couldn't recreate your issue in our lab but we have some suspicions. Check what is your CPU usage during recording.

COD games close themselves when Action! hook up to them. We have contacted Activision but they refused to help us.
Try adding those games to exclusions and record them using Screen mode. You might need to run those games in borderless fullscreen.
Feel free to report this problem to Activision support.

Please go to Settings -> Audio and make sure to enable "Record system sounds" option and disable "AUX audio feature".

We have the problem. It was related with the NVENC HEVC hardware acceleration that your graphics cards do not support. Problem will be fixed in next release. We are rolling out new version today. Please let us know if your problem is resolved.

Looks like your hardware acceleration is not working. Disable hardware acceleration and see if you get similar results.
