I have this problem in all games, but Outlast gives me the most problems.
OS: Windows 8
Video Card: NVIDIA GTX 780
Current Driver version and date: 347.09, 23/12/2014
Memory: 16 GB
Monitor Resolution: 1920x1080
Recording mode: Game
Video Type: MP4 NVIDIA NVENC
FPS: 30/50/60

The problem with this game is the biggest, because when I choose 30FPS, ShadowPlay tells me the game is being displayed with 60FPS, but I can feel it that value is definitely lower (about 30), I don't have this kind of problem in other games (but audio is desynchronized in every game).

For me it happens mostly on Dragon Age: Inquisition, and only in cinematics, recording at 1080p 30 fps. I think it has to do with that the cinematics on Dragon Age: Inquisition are 30 fps locked, but not the rest of the game.

Could you please check what's your computer screen refresh rate and what's the sound type of your video? If you will playback your video in Action! player press "i" for more details about your video.

Refresh rate: 60 hz
Audio type: Linear PCM (Stereo, 48000Hz)

BTW, I record the mic into separated audio track and that track is not desync, just the game's one.

Another fact: when I record Dragon Age: Inquisition, files appear as "Video 07-01-2015 18-09-59" and not "DragonAgeInquisition 07-01-2015 18-09-59".

AAC 48000 Hz. I also record microphone on separate track, but for me mic recording is desynched too.
