Action crashes when done recording and can't open file

Hi there.
I used a different setting this time around to record my videos. I used to use MP4 with Intel QuickSync but every 4th video would make Action crash and then I would have to go to Task manager to close the program. I then open it back again and I see the file is there but I can't open it with the Action video player. It says "Failed to open input file". It also says that the file is at varied sizes... Sometimes its 0 bytes or its 300 MB while it should be a bit more than 1 Gig.
Anyway, I tried the MP4 (AMD APP) settings and it was working flawlessly until just now. I just lost a 10 minute video and got the same error (Failed to open input file). Also, this time around, my game also crashed and got disconnected at the same time. The file is 1.5 gigs though. So I know it recorded it but I can't seem to open it... So I believe it is lost to me. This video was like the 27th video I recorded, which I cannot open or do anything with. I am going to continue recording again and hopefully the crash doesn't occur again. Majority of my videos are around 10 minutes each, while 2 or 3 of these videos are maybe more than 10 minutes, like 20 minutes or 15 minutes.
I did get a suggestion to rein-stall and I just did that... Now, hopefully, I don't get this error again, because I cannot re-record my gameplay footage since then I would have to restart all over again and I have already done that once.
If there is any way to open the file, please let me know.
