MP4 recording buggy -see video sample-

Hello , I cant find the prober word , so I used "buggy"
I had this problem for like 3 mth. (all my drivers is up to date, yes even my chipset)
mp4 recording - any video size- any frame rate -any bit rate- windoed or full screen- ONLY in mp4 not in avi
Well my English is not the best.
But I hope a picture / video says more than words.
