action not recording properly

I have used this for awhile now but this problem recently came up out of nowhere. When I open a video in adobe premiere it works fine for a second but then sound cuts and the video speeds up like 400x the normal rate. If i open in windows media it works for a second then says cannot read file. Help?

Do you record in MP4 or AVI?
